How one layer of a bill is priced
The pages above price whole procedures. Many bills, though, are confusing because of one layer inside them: the visit level an emergency department assigned, the anesthesia time, the lab panel, or the pathology read on a biopsy. Each of those layers has its own Medicare payment logic and its own dated anchor, and a patient can ask about it separately from the rest of the bill. The guides below take one billing layer at a time, show the CMS figures behind it, and explain which question to put to the billing office.
