Procedure quote comparison worksheet

This worksheet holds only numbers you obtained yourself. Call each facility, ask for the cash price, a Good Faith Estimate, or your insurer negotiated estimate for one exact CPT code, and type each answer in. The worksheet lines the facilities up side by side and places the verified Medicare anchor next to your numbers for order of magnitude. It invents no prices.

Your data stays in your browser. This worksheet runs on this page only. Nothing you type is sent, saved on a server, or shared. If you close the page, the entries are gone. Use the print button to keep a copy.

Short answer: get a number first, then compare it here.

This worksheet compares only quotes you obtained yourself. It does not look up prices. Before you call a facility, check a price in the official price-check resources directory: 24 official resources (5 federal, 15 state, and 4 insurer tools), listing official price-check resources only and no facility prices.

2. Enter each quote you obtained

One row per facility. Keep the three price types separate: a cash price, a Good Faith Estimate, and an insurer negotiated estimate are three different numbers. Note what each amount includes, because a low number that excludes the professional fee is not the all-in price.

3. Side by side comparison

Rows appear here as you type, sorted by amount from lowest to highest. The anchor column is simple arithmetic: your entered amount minus the Medicare PFS anchor shown above. It is there for order of magnitude only. The anchor is a Medicare payment, not a price any facility offered you.

How to get numbers worth entering

  • Ask for one exact CPT code in one exact setting. A quote for a different code will not match the bill.
  • Ask what the amount includes: facility, professional, anesthesia, and pathology are separate layers. Write the answer in the notes field.
  • If you are uninsured or paying cash, ask for a Good Faith Estimate in writing. The Good Faith Estimates guide explains the timing rules and the dispute process.
  • Check the official tools in the price-check resources directory before you call, so you know what a resource average or a Medicare anchor looks like for the same code.
  • Use the billing checklist on the call, and confirm the all-in price in writing before service.

What this worksheet does not do

It does not look up prices, contact facilities, or estimate what a facility will charge. Every amount in the table was typed by you. Medicare anchors are read from this site checked-in data file (src/data/procedures.json) and are shown with their source, year, and verified date. They are not blended into your entered amounts and they are not a prediction of your bill.
